 Cigna Medical Group (CMG) has been honored by The Arizona Partnership for Immunization (TAPI) with the organization's annual "Dr. Daniel T. Cloud Outstanding Practice Award." Cigna Medical Group was recognized for its leadership and excellence in immunizing Arizona children - reaching 90 percent coverage levels. Winner - AZ Business Magazine Health Care Leadership Award - Institutional or Educational Program, in the Institutional or Educational Program category. Our Home-Based Care Program was created in 2004 specifically to help patients who were not returning to their doctor after a hospital visit. This was an important gap that needed to be filled and our Population Health Management team has been providing the Home-Based Care Program ever since.
The program has a team of physicians, PAs, NPs, LPNs and social workers who visit patients in their homes to provide medical care.They monitor vital signs, provide treatment, review patient prescriptions, help them take medications, evaluate nutritional needs and more.And all activity is recorded in the Electronic Health Record, available to the primary care physician.Since the program started, there has been a 30 percent decrease in acute hospital admissions.
