Sports physicals are a time to review the child's health history, ensure immunizations are up-to-date, and discuss any changes in their health - even if it something that seems insignificant such as bruising. Service includes:

• Review of medical history
• Review of immunization history (patient to provide)
• Eye exam
• Urinalysis (if required)
• Printed assessment for records

* All services are provided by qualified physician assistants and nurse practitioners who are under the supervision of a CMG CareToday Medical Director. Further diagnostic tests may be required based on findings for an additional fee. Children up to 18 years of age are eligible.
