What do I need to include in a new group submission?
Please reference the checklist in the California Small Group Underwriting Guidelines (PDF). The forms required are located in the document library.
What are the Submission deadlines?
We ask that all completed paperwork be delivered to our sales office by the 15th of the month. CIGNA will, however, accept completed paperwork until the 5th day of the effective month.
How do I get a rate quote?
You may email quote requests and the census information to info@aaqhc.com.
Quotes can also be obtained through Quotit and Health Connect. You may also contact us by telephone at 800.669.8799, Option 2.
When will renewal rates be available?
Renewal rates will be mailed to the Broker and the Employer Group approximately 60 days in advance of the renewal date.

Who should I contact about claims issues?
Call CIGNA at 800.224.6224.
