What do I need to include in a new group submission?
You will need to supply everything on the Case Submission Checklist (PDF). The forms required are located in the Document Library.
What are the submission deadlines?
Complete group submissons must be turned in to our Sales Office by the following dates in order to be submitted to Underwriting:
1st of the month effective dates: 10th day of the prior month
15th of the month effective dates: 30th day of the prior month
How do I get a preliminary rate quote?
You may download our current rate model from our rate model page or you may contact one of our General Agents, if you prefer they can prepare one for you.
How much can final rates vary from the preliminary rates?
Once Underwriting has reviewed all documentation and medical information, final rates may be increased up to the following:
- North Carolina: 1 employee - 25% 2-24 employees 67%, 25+ employees 47%
- South Carolina: 1 employee - 25% 2-24 employees 67%, 25+ employees 52%
When will renewal rates be available?
Renewal rates are faxed to the broker 45-60 days before the renewal date. They are also mailed to the main contact at the group.
Who do I contact about billing issues?
You should contact Eligibility/Billing at 800.603.0125 for both North and South Carolina.
Who do I contact about claims issues?
You should contact Member Services at the numbers below. Keep in mind, you will need the member number, member’s date of birth, and the member’s zip code.
