About the Plan
The CIGNA HealthCare of Arizona Individual Plan offers a simple-to-use, comprehensive health plan built to provide you and your family with the comfort and peace of mind that comes with dependable health coverage.
With the CIGNA HealthCare of Arizona Individual Plan, you can expect quality health care and affordable coverage.
The Individual Plan is available to:
- Individuals 18 years of age or older
- Families with children up to 18 years of age or 23 if a full-time student
- Children without an adult subscriber who are at least 3 months of age
You must reside in one of the defined Service Areas for at least nine (9) months per Plan Year:
Phoenix Service Area – covering Maricopa County and the city of Apache Junction, AZ
Tucson & Southern Arizona Service Area – covering Cochise, Graham, Greenlee, Pima, Pinal and Santa Cruz counties, AZ

Provider Network Options
The CIGNA HealthCare of Arizona Individual Plan offers two provider networks:
- CIGNA Medical Group Provider Network – available to members in the Phoenix Service Area
- Arizona Provider Network – available to members in the Phoenix or Tucson & Southern Arizona Service Areas
When you enroll, you select a Primary Care Physician (PCP) for yourself and one for each covered member of your family. Your PCP provides or assists in coordinating your medical care whether it's preventive, specialty care or treatment for an illness or injury.
For CIGNA HealthCare of Arizona Individual Plan members, two provider network options are available:
CIGNA Medical Group Provider Directory – The CIGNA Medical Group, features 400 doctors and support staff that provide primary and some specialty care at health care centers throughout the Valley. Additional specialty care is provided by referral to specialists in private practice. Most health care centers offer lab, X-ray and pharmacy services in one convenient location. Some centers also offer Vision and Hearing services and Urgent Care. CIGNA Medical Group also offers nutritional counseling, wellness classes, and chiropractic and massage services at select locations.
Arizona Provider Network Provider Directory – The Arizona Provider Network includes a wide selection of private practice physicians in the CIGNA HealthCare of Arizona contracted network. If you select this network, you will select a PCP from and all of your specialty care will be coordinated through this network.
For the most up-to-date provider listing, just log on to myCIGNA.com and select "Provider Directory" at the top of the page. Type in your zip code and then select "AZ - Arizona" for the Arizona Provider network or "AZ - CIGNA Medical Group" for the CIGNA Medical Group from the "Network (HMO) Plans or Point of Service (POS) Plans" drop down list. Enter what kind of physician you're looking for and click "search."

How to Apply
You can apply for the CIGNA HealthCare of Arizona Individual Plan in one of two ways:
- Apply on line – complete the enrollment and email to AZIndividualPlan@cigna.com
- Print and mail – complete, print and sign both forms and mail to:

Your application will be reviewed and you will be notified of your enrollment status within 10 business days.
Applications approved through the 10th day of the month can become effective retroactively to the first day of that month. For example, the effective date of coverage for an application approved June 8th would be June 1st.

What is the difference between the CIGNA Medical Group and the Arizona Provider Network?
By selecting the CIGNA Medical Group, you'll receive all of your primary and specialty care through the CIGNA Medical Group (CMG), a network of full-service health care centers and other medical facilities located throughout the greater Phoenix area. The CMG provider network is available to members in the Phoenix service area only.
The Arizona Provider Network includes a wide selection of private practice providers in the CIGNA HealthCare of Arizona contracted network. The Arizona Provider Network is available in the Phoenix and Tucson/Southern Arizona service areas.
Can I select a PCP from the CIGNA Medical Group for myself and a PCP from the Arizona Provider Network for my spouse?
Yes, the benefits and services provided are the same for both networks, however, the monthly premium amount differs. If you choose different networks for family members, two separate applications must be completed.
What should I do if I want to change my Primary Care Physician?
Changing your PCP is easy. You can 1) call CIGNA HealthCare Member Services at 1.800.CIGNA24 (1.800.244.6224), or 2) you can complete the and follow the instructions or 3) log on to . Please note that your monthly premium rate will be adjusted if you change to a Primary Care Physician in a different network. For example, if you change your physician from one in the Arizona Provider network to one in the CIGNA Medical Group network, your monthly premium rate will change
Is there a waiting period for maternity benefits?
In order to be covered under the CIGNA HealthCare of Arizona Individual Plan, delivery must occur 12 months after the effective date of coverage.
Will my monthly premium change if I have a birthday that puts me into a different age category?
Your monthly premium will automatically be adjusted the month following a birthday that puts you into a higher age category.
Am I covered outside of the service area and outside of the country?
For emergencies, you're covered out of the service area – even outside of the U.S. For all non-emergency/non-urgent care, you should receive care from within your designated provider network.
Which hospitals can I use?
As a CIGNA HealthCare of Arizona member, you have access to a large network of hospitals throughout the state.
For the most up-to-date hospital listing, just log on to www.cigna.com and select "Provider Directory" at the top of the page. Select "Hospital" under "Type of Provider" and type in your zip code. Then select "AZ - Arizona" for the Arizona Provider Network or "AZ - CIGNA Medical Group" for the CIGNA Medical Group from the "Network (HMO) Plans or Point of Service (POS) Plans" drop down list.
