CIGNA Medicare Select Plus Rx - a Medicare Advantage HMO medical plan with Part D prescription drug coverage - provides predictable, affordable coverage beyond what you get with Original Medicare. You can rest easy knowing you have the security of reliable medical coverage and rich benefit design with minimal out-of-pocket expense. It's a $0 premium plan that includes prescription coverage, even through the gap.

As a CIGNA Medicare Select Plus Rx customer, all of your primary care and most specialty care will be provided through the Cigna Medical Group network, with more than 25 locations throughout the Valley. Cigna Medical Group facilities are conveniently located throughout the greater Phoenix area and many locations offer primary care, specialty care, lab, X-ray and pharmacy all in one location.

Save money.

The CIGNA Medicare Select Plus Rx plan offers many services including preventive care services at no charge. And PCP and contracted urgent care visits are just $5 per visit, inpatient hospital is covered for just a $500 copay and a dental supplement is available for just a $17 monthly premium.

Extras

As a CIGNA Medicare Select Plus Rx customer, you're eligible to enroll in Golden Vitality, a wellness program designed to educate and encourage you to make positive lifestyle choices. The program is FREE and all participants are eligible to receive either a $200 gym membership reimbursement to the gym of your choice or an at-home fitness kit.

Eligibility

The Medicare Annual Enrollment Period runs from November 15 through December 31st each year. During this time, you may generally switch from one Medicare Advantage plan to another. Certain exceptions apply for individuals that may wish to enroll outside of this enrollment period.

You may be eligible to enroll in a CIGNA Medicare Select Plus Rx plan if:

• You are entitled to Medicare Part A and enrolled in Medicare Part B.
• You live in the service area of Maricopa County or certain zip codes within Apache Junction or Queen Creek, Arizona
• You do not have End Stage Renal Disease (ESRD), unless you are a member of our plan and have been since your dialysis began

Additional enrollment criteria for those individual wishing to enroll in the CIGNA Medicare Select Plus Rx - Dual plan:

• In addition to the above criteria, you must also be eligible for Medicaid benefits through the Arizona Health Care Cost Containment System (AHCCCS).

If you have access to the web, you may use the web tools on www.medicare.gov by selecting either "Compare Health Plans and Medigap Policies in Your Area" or "Compare Medicare Prescription Drug Plans." You may be able to get extra help for your prescription drug plan costs. To see if you qualify for getting extra help, call
