Take Advantage of CareAllies Wellness Resources

The CareAllies program offers many resources to help you improve your health. All salaried employees, retirees, surviving spouses and covered spouses/same-sex domestic partners enrolled in a Ford PPO plan are encouraged to participate. Completing the Wellness Criteria allows you to receive the Enhanced Benefit Level, with lower deductibles and out-of-pocket costs.

Action Required: To receive the Enhanced Benefit Level for 2014, you must take action by March 31, 2014. You can complete your physical exam anytime after October 1, 2013. Review this Brochure for complete details and see the Flyer for step-by-step instructions. The Flyer and Personalized Member Qualification form will be mailed to your home in late December. Additional copies of your Personalized Member Qualification Form can be obtained by clicking on the My Health Assessment link on this page which will take you to the CareAllies home page. Simply click on the link under Quick Links entitled "Check Status of Wellnes Requirements".

You can complete the online health assessment beginning October 1, 2013. The Health Assessment helps identify any health issues you may have - or may be at risk for.

Tobacco Cessation
If you are an Active Ford Employee and you or your spouse/same sex domestic partner certified during open enrollment to be a tobacco user but you are willing to quit, you can avoid the $50 per month tobacco surcharge by enrolling in the tobacco cessation program by no later than March 31, 2014. For more information about the tobacco cessation programs, click on the Lifestyle Management Programs' link on the right side of this page.

Important Note
