Take Advantage of CareAllies Wellness Resources
The CareAllies program offers many resources to help you improve your health. All salaried employees, retirees, surviving spouses and covered spouses/same-sex domestic partners enrolled in a Ford PPO plan are encouraged to participate. Completing the Wellness Criteria allows you to receive the Enhanced Benefit Level, with lower deductibles and out-of-pocket costs.
Action Required: To receive the Enhanced Benefit Level for 2015, you must take action before March 31, 2015. If you had a physical after October 1, 2014 you may use those results on this year’s Member Qualification Form (MQF) and have your doctor sign it.
To obtain a Non-Personalized copy of your Member Qualification Form (MQF) please click here MQF.
You will be able to check your Wellness Status after January 2, 2015. To do so simply click on the link under Quick Links entitled "Check Status of Wellness Requirements."
Take my health assessment
You can complete the online health assessment beginning October 1, 2014. The Health Assessment helps identify any health issues you may have - or may be at risk for.
If you are an Active Ford Employee and you or your spouse/same sex domestic partner certified during open enrollment to be a tobacco user but you are willing to quit, you can avoid the $50 per month tobacco surcharge by enrolling in the tobacco cessation program by no later than March 31, 2015. For more information about the tobacco cessation programs, click on the Lifestyle Management Programs' link on the right side of this page.
Please note: All Employees, Retirees and Spouses/SSDP must first register prior to taking the Health Assessment.

Completion of the Member Qualification Form and the online health assessment are two separate requirements. Both requirements must be met to maintain or qualify for the Enhanced Benefit Level. See the Checklist for details.
