Find a Provider in Cigna's Open Access Plus Network

This is the Cigna network for the State of Arizona EPO health benefit plan. We work with most doctors in Arizona and are adding new health care providers all the time. You'll find all the major hospitals as well, including Mayo Clinic, Banner, St. Joseph's, John C. Lincoln hospitals, UMC and TMC. What if my provider is not in the Cigna network?

• Nominate a Provider

To nominate a doctor or other health care provider to the Cigna Open Access Plus network, complete the Provider Nomination form (Word 31k).

• Apply for Transition of Care

If you are receiving ongoing treatment for certain conditions from providers who are not part of the Cigna network, you can apply for Transition of Care benefits to have Cigna arrange to cover these services. Read more and submit this form (PDF 47k) to begin the application process. The form will be processed once you enroll in a Cigna plan and we receive eligibility information.
