Claim Information & Resources
This section outlines how the disability claim process works and what medical information we'll need from you to help us make claim decisions. We've also outlined the information we'll need from your patient to begin the claim process below.
If your patient or the patient's family member has not yet filed a disability claim with us, they can do so by calling our Intake Center at 1.800.36.CIGNA (24462), or by completing an online claim form on the CIGNA Web site (www.cigna.com). They should have the following information handy when submitting claims online:
- name, address, and telephone number of treating providers (you and/or hospital);
- last day worked, or date of injury or onset of illness;
- dates of the first, last and next visit with you;
- supervisor's name, telephone number and email address; and
- other income sources (for example, Workers' Compensation, Social Security, state disability or pension). This information may affect your patient's disability benefits.
