Welcome to the Physicians' Disability Toolkit SM

The Physicians' Disability ToolkitSM is designed to help health care professionals like you, and your office staff, understand more about how the disability claim process works and your important role in helping us make claim decisions.

Out-of-work patients you treat for illness or injury are not automatically approved for disability benefits. This Toolkit helps explain what qualifies as disability and how information from you and your patients helps us make disability decisions appropriately.

Use the menu on the left to learn more about basic disability information, how you fit into and contribute to the claims process, forms and contacts you'll need to provide us with medical documentation, plus common terms we use and questions we hear from other physicians.
