Locating a Network Dentist
If you are enrolling or thinking about enrolling, it's quick and easy to search for a participating network dentist.
Simply go to our On-line Dentist Directory at www.cigna.com to search for a convenient CIGNA Dental Care HMO network dentist by name or zip code.
To enroll:
Refer to your enrollment worksheet. Should you have additional questions, contact the U.S. Bank Employee Service Center at 1.800.806.7009.
Choosing a Network Dentist
- Fill out this Dental selection form and send or fax it to CIGNA Dental so we can assign you to your selected network general dentist.
- If you do not select a dentist, we will choose one for you based on the closest available dental office to your home zip code.
- Once you receive your ID card in the mail, you can call Customer Service at 1.800.CIGNA24 or go to www.mycigna.com to change your dentist at any time. Transfers will be effective the first of the month following the request.
Once Enrolled:
Log onto www.mycigna.com for your personal benefit information and additional plan materials.

Additional Dental Health Information
Research supports a relationship between periodontal disease and diabetes, heart disease and stroke, osteoporosis, respiratory problems, as well as adverse birth outcomes. These educational flyers help promote good oral health which may lead to better overall health.
In addition, our Healthy Rewards® program* is available to all dental members with an ID card. This program provides discounts on services like acupuncture, chiropractic services, massage therapy, eye exams, laser vision correction, vitamins, herbal supplements, anti-cavity products, and non-prescription health and beauty products. There are no added membership fees or limits to the number of services members can use.
*Some Healthy Rewards are not available in all states.
