Community Health Worker Pilot
The Community Health Worker pilot is collaborating with Harriot Home Health Services (HHHS), a local business, to expand its work in the neighborhood. HHHS is a full service state-licensed, Medicare certified and Joint Commission accredited home health agency. Community Solutions (CS) is assisting in this work to demonstrate the critical role the Community Health Worker play as a sustainable option for improved neighborhood health. Other important partners for this pilot include St. Francis Hospital, Capital Workforce Partners and representatives from the State of Connecticut. CS is engaged with Systems and Management Consulting Partners, LLC, and their participation is crucial in tracking social determinants of the health of the community, as well as enabling case management services.
The Swift Factory Vision
Located in the heart of Northeast Hartford, the 65,000 square-foot former Swift gold-leafing factory has been gifted to Community Solutions with the vision of turning it into a community hub. The vision includes activities to support job training and local production of food related businesses such as a cafe and a commercial kitchen. Goals also include providing nutrition and health education to the community, as well as connect with a public health network to include an on-site health facility.
Employment Coaching and Vocational Case Management
Employment coaching is taking place through a “Living to Work” toolkit designed for Community Solutions staff. It includes resources for people looking for employment that may have a criminal record or be on Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI).
