1 Applies to diagnostic testing for customers in the United States who are covered under Cigna’s employer or union-sponsored insured group health plans, insured plans for US-based globally mobile individuals, and Medicare Advantage and Individual and Family Plans (IFP). Cigna will also administer the cost share waiver for self-funded group health plans, although some self-funded plans may choose to opt out of the waiver option. Coverage criteria applies. Effective through at least January 21, 2021.

2 Employee assistance program services are in addition to, not instead of, your health plan benefits. These services are separate from your health plan benefits and do not provide reimbursement for financial losses. Program availability may vary by plan type and location, and are not available where prohibited by law.
