First-time Credentialing

How to Join a Behavioral Network

  1. Confirm you meet the minimum credentialing requirements
    • A license to practice independently (without supervision) in the state in which you are providing services
    • Professional liability insurance with minimum limits of:
      • $1 million/$3 million for Physicians and Psychiatric Nurses with prescriptive authority, or
      • $1 million/$1 million for all other providers; exceptions include:
        • Texas: the minimum recommended amount of professional liability coverage for providers in Texas is $200,000 per occurrence and $500,000 aggregate
        • Florida: the minimum recommended amount of professional liability coverage for providers in Florida is $100,000 per occurrence and $300,000 aggregate
        • Providers in states which regulate a patient compensation fund for insurance coverage, or which regulate the amount of insurance a provider must carry, should meet at least the state's requirement if they do not meet Behavioral Health's minimum requirements
    • For clinics only:
      • Multi-disciplinary staff must include 10 providers. The provider mix must include at least:
        • One prescriber
        • One doctorate level provider who can conduct psychological testing
        • Three or more master's level providers
      • All clinicians must be state licensed to practice independently without restrictions or supervision
      • Clinic must be willing to cooperate with Cigna's credentialing and recredentialing processes, including utilization of the Council for Affordable Quality Healthcare (CAQH) Universal Credentialing Database
      • Clinic must have centralized intake, or the ability to conduct a full intake at each location (if there are multiple locations)
      • All billing must be submitted under the clinic name and tax ID number
  2. Fill out an application
    • Individual practitioners fill out the Behavioral Health Provider Application
    • Individual practitioners in Maryland are not required to fill out the application. Email us at BehavioralHCPEnrollment@evernorth.com with your request to participate and include the following information for identification purposes:
      • First and last name
      • Middle initial
      • CAQH ID#
      • Tax Identification Number (TIN)
      • CBP id
      • Individual NPI
      • National Provider Identifier (NPI)
      • License type (LCSW, LPC, LMFT, MD, LP, etc.)
      • Primary mailing address
      • Service locations
      • Phone number (for contracting/credentialing purposes)
    • Individual practitioners in Minnesota, North Dakota or western Wisconsin, fill out the HealthPartners Behavioral Health Contract Application available at the HealthPartners Provider portal
    • Clinics, fill out the Screening Application for Behavioral Health Clinics
    • Facilities, call Provider Services at 1 (800) 926-2273. Please note the Behavioral Health Provider Application cannot be used by facilities seeking to join the network.
  3. Welcome to the network!
    If you are approved and credentialed, you will receive an email within 20 business days (15 for individual practitioners in Maryland) letting you know that you have been approved and when your effective date is.

Recredentialing

When you need to get recredentialed, Cigna will send you an email asking you to verify your CAQH information is updated, and re-sign it. You will receive up to 3 reminder emails. If you do not log into CAQH to verify and sign your information, it will result in termination with the Cigna network.
