Eligibility Criteria

  • Individuals age 64 1/2 or younger on the assigned effective date
  • Primary applicants must be age 19 or older; children under the age of 19 may only apply as a dependent on a family plan with one or both parents
  • You must be a resident of the state of Georgia
  • You must be a legal U.S. resident and must reside within the U.S. for six consecutive months prior to applying for coverage

Dependent Eligibility Criteria

  • Your lawful spouse
  • Your children, stepchildren, or adopted children who have not yet reached age 26
  • Your own or your spouse's children with continuing mental retardation or physical disability, may continue to be covered beyond age 26 if enrolled prior to reaching age 26 and are unmarried. Proof of disability and dependency is required
