- For an affordable monthly premium, you select a base policy with a maximum benefit amount* of $5,000 to $100,000.4
- Policies can be issued from ages from 18 to 99 and can include coverage for your dependents*
- The base policy covers heart attack, stroke, heart transplant, angioplasty, coronary artery bypass surgery, and other heart-related procedures listed in your policy.
- You can choose to add more benefits for an additional premium, such as a restoration coverage for a later heart attack or stroke diagnosis or procedure, Lump Sum Cancer Insurance, and other riders for specific situations.5

How can I spend the money?
How you use the cash benefit is up to you. We pay regardless of any other insurance you may have, and we pay directly to you, or your designee, to use any way you like. Use the lump sum benefit to help cover:

- Unexpected expenses such as travel
