    What are Medical Network Plans?

    These health plans offer you cost-efficient access to a local network that's limited to quality doctors, specialists and hospitals, right where you live, and who understand the needs of your community.

    Medical Network Plan

    • In-network primary care provider (PCP) required for yourself and your covered dependents.
    • Referrals needed from your PCP for specialists, except for OB/GYN services. 
    • Out-of-network services will only be covered in emergencies*.
    • Prior authorizations (pre-approval) may be needed for hospitalizations and some types of outpatient care.
    • Plans include global emergency and urgent care coverage*, 24/7.

    Medical Network Point of Service Plan

    • A primary care provider (PCP) is required for yourself and your covered dependents.
    • Referrals needed from your PCP for specialists, except for OB/GYN services. 
    • Out-of-network services are covered but you will have higher out-of-pocket costs and you may need to file a claim.
    • Prior authorizations (pre-approval) may be needed for hospitalizations and some types of outpatient care.
    • Plans include global emergency and urgent care coverage*, 24/7.
