Cigna Joins Initiative in Southeastern Pennsylvania to Address Chronic Disease
Initiative includes Patient-Centered Medical Home Model
BLOOMFIELD, Conn., May 16, 2008 - Cigna today said it is participating as one of the payers in a Pennsylvania program that adopts the patient-centered medical home model as part of a wider initiative to improve health care for people with chronic diseases. The initiative was established through the recommendations of the Pennsylvania Chronic Care Management, Reimbursement and Cost Reduction Commission created by Governor Edward G. Rendell. An important milestone in the program rollout was reached this week with the implementation of the first regional learning collaborative in southeastern Pennsylvania.
"We are pleased to be joining primary care doctors and nurse practitioners in the Philadelphia area in this coordinated effort to enhance care for the people Cigna serves," said Dr. Jeff Kang, Cigna's chief medical officer, himself a primary care physician. "We believe the patient-centered medical home model is a promising approach that facilitates moving from a fragmented health care system to a more coordinated system, improving satisfaction for our customers and their primary care providers."
The Pennsylvania regional learning collaboratives were established to assist primary care practices in redesigning their practices to better support patients with chronic diseases. The patient-centered medical home model of care is designed to improve quality and lower medical costs by providing patients with a comprehensive, coordinated approach to primary care. Patients have access to enhanced care coordination, communications, appointment availability and education to help them navigate a fragmented health care system, while physicians receive additional reimbursement for providing these enhanced services and supportive infrastructure such as additional patient counseling.
Primary care practitioners participating in the southeastern Pennsylvania program will be paid for the medical services they provide and will be reimbursed an additional amount for enhanced services such as care management for patients. They will also receive the assistance of a practice coach to help them establish new workflows and have use of a patient registry.
In addition to its support for the program in southeastern Pennsylvania, Cigna is an active participant in the Patient-Centered Primary Care Collaborative (PCPCC) a coalition of more than 40 major employers, consumer groups, organizations representing primary care physicians, and other stakeholders who have joined to advance the patient-centered "medical home." Cigna is supporting a patient-centered medical home pilot program in Colorado and will also launch a Cigna-specific medical home pilot in the Northeast this year.
